Privacy Practices (HIPAA): 

HLC Therapy Group, LLC requires consent to care and treatment as recommended by the therapist/s as is
necessary in his/her clinical judgment. HLC Therapy Group, LLC requires consent to use or disclose, for the purpose of carrying out treatment, payment, or health care operations
(TPO), all information contained in the patient records. This may include sharing/exchanging information with physicians or other health care or educational professionals who are caring for my child. HLC Therapy Group, LLC requires authorization to use information in any way that is outside
of the allowable reasons under the HIPAA regulations if deemed appropriate by the therapist, for which an accounting of disclosures will be kept.
